Trials in Sheltered Water
A hull is proved in calm water with a support vessel alongside and instruments recording everything. That is how you get a measurement. It is not how the ship will be used — and a result obtained under those conditions describes those conditions.
Conditions of measurement and conditions of use are different, and the difference is usually unstated. These three keep it stated.
Where we report a trial result, we report who was in it — the age range, the health status, what excluded a volunteer. A finding is a statement about the people it was measured in, and a page that omits them has quietly promoted a specific result into a general one.
Whether you resemble the people in a given trial is not a judgement to make from a web page. Your medications, other conditions, and age are precisely the variables that decide it, and a clinician holding all three is the only one positioned to say.
A result obtained under supervision, with adherence measured and harms actively sought, is not a result about unsupervised use. Where we know the conditions we state them, and where a paper does not report them we say that too. We sell no supplement, hold no affiliate arrangement, and publish no testimonials.
This argument cuts against optimism, not against care. Noting that trial conditions flatter a product is a reason to expect less from it, not a reason to distrust the medicine practised on you — clinical treatment is chosen for your circumstances rather than measured in a cohort. And nothing here suggests that being unlike a trial population means a recommendation does not apply to you. That inference requires someone who knows both, and it is exactly the wrong thing to conclude alone.
